If you received a proficiency testing kit and you are no longer testing patients at your facility, there are a few things you should do:
- Discard the samples as you would any patient sample. Do not attempt to send the samples back to API. We cannot reimburse shipping costs and we cannot reuse the samples.
- Ensure that the testing is marked as discontinued and complete the submission process. This must be completed to avoid receiving a "failure to participate" on an evaluation. You may do this by logging on to Paperless Proficiency Testing and submitting this online or you may make a note on the Comments page (included in your kit's paperwork) and you may fax it to us at 855-900-6119.
- Marking the testing as discontinued will not result in any order changes. If you need to cancel an entire program from your order, you must contact Customer Service at CustomerService@api-pt.com or 800-333-0958.
