If your laboratory has changed from one accrediting agency to another, please email the details of the change to Customer Service at CustomerService@api-pt.com. You may also email a completed Data Release Form to request the change. A blank Data Release Form is available to view and download in the Getting Started FAQ section. It can also be accessed on the API website under the Catalog heading.
API sends enrollment and performance information to accrediting agencies. It is important that all accrediting information is accurate to ensure proper reporting.
