When you submit results online, you enter the name of the laboratory director and the names of all personnel who performed testing for that event. Once the results are transmitted, an electronic Attestation Statement is generated. You can:
- Print the electronic version, which shows a timestamp in the upper, right-hand corner of the page, have the personnel sign it, and save it with your documentation OR
- Use the Attestation Statement that is included in your printed paperwork (either received in your kit or printed from the API website, depending on your paperless settings). Complete the information by hand, have all of the personnel sign it, and save it with your documentation. If you need a new copy of this Attestation Statement, you can reprint a copy by clicking the Instructions & Worksheets option on the Current Events page.
The only time you should mail the signed Attestation Statement to API is if you are mailing all of your worksheets.
